How to Use the Name Field

Name Field Feature
A quick and robust way to collect personal identifiers with Simple and Extended format support.
Step 1: Add the Name Field
Drag the Name Field from the Right Sidebar and drop it anywhere into your form canvas.
Step 2: Collect More Than First & Last Name
Click on the Name Field inside the builder to open its settings. By default, only First and Last Name are collected.
Step 3: Enable Sub-Fields
Under the General tab in the Right Sidebar, you can selectively toggle the visibility of extra sub-fields like Prefix, Middle Name, and Suffix. Formpify will automatically adjust the grid layout responsive to your changes.
